What Your First TRT Appointment Involves
Most men who come to us from San Antonio have already been told their testosterone is 'within range'. Here is what we do differently, and what the first visit looks like.

We start with your symptoms
Fatigue, loss of strength, low libido, poor concentration and low mood — we go through what you are actually experiencing and how long it has been going on, before anyone looks at a number.

Labs drawn on site, not referred out
Bloodwork happens in the clinic during your visit. You are not sent to a separate lab and asked to come back, which is where most of the delay in conventional referral pathways comes from.

A full hormone panel, read properly
Total and free testosterone, SHBG, estradiol and the wider metabolic markers. 'Normal' on a total testosterone result frequently hides a free testosterone level that explains exactly how you feel.

Dr. Jaqua reviews your results and tells you whether treatment is clinically justified. If it is not, we say so and look at what else could be behind your symptoms.

Why 'Normal' Labs Often Are Not the Whole Story
Reference ranges are wide, and they are built from a general population rather than from men your age with your symptoms. These are the factors that change how a result should be read.
Free vs total testosterone
Total testosterone can sit inside the reference range while free testosterone — the portion your tissues can actually use — is well below optimal. Testing only the total misses this entirely.
SHBG changes the picture
High sex hormone-binding globulin locks up available testosterone. Two men with identical total results can have very different free levels because of it.
Draw timing matters
Testosterone peaks in the morning and declines through the day. An afternoon sample can read low enough to mislead in one direction, or mask a deficiency in the other.
Estradiol balance
Testosterone aromatises to estradiol. Managing that ratio is a large part of why supervised TRT feels different from simply being handed a prescription.
One result is not a diagnosis
Confirmation on a repeat draw is standard practice. A single reading on a single morning is not enough to start lifelong therapy on.
Symptoms carry equal weight
We treat men with confirmed low testosterone who also have symptoms. Numbers alone, or symptoms alone, are not sufficient grounds for treatment.
What TRT will not fix
Testosterone therapy is not a treatment for poor sleep, untreated depression, thyroid disease or the effects of an unmanaged metabolic condition. Those can produce identical symptoms, and starting TRT without excluding them means treating the wrong problem. Part of the initial workup is ruling them out.
Fertility considerations
TRT suppresses the body's own testosterone production and can reduce sperm production while you are on it. If you are planning to father children, say so at the first appointment — it changes what we recommend.
